Workers Compensation
As of July 1, 2017, our District switched to a new Worker’s Compensation vendor (United Heartland). While the procedure remains the same, there are changes to the paperwork that must be completed when an employee is injured while at work. Please dispose of any old forms you may have and use the following forms.
Please remember that when an employee is injured at work, no matter the severity of the injury, the employee must notify their supervisor immediately and complete an Employee Injury Report. The injured employee and their supervisor must then meet and complete the Supervisor’s Report of Accident. Both forms should be completed and returned to the Human Resources Department immediately. If the employee is planning to seek medical treatment, they must submit to Human Resources a Return-to-Work document from their healthcare provider prior to returning to work.
